UX design for new item mapping - look and feel only

Charlotte is now officially the PO for Fast Add.  

Time frame for development - Preference is Q1 2020  - Charlotte to speak w/Mike Gorell & Andrea to speak with Holly M to advocate for UXPROD-1057

•  Additional needs:
  • Radio button to differentiate between items to be referred to cataloging for update or not to be referred (faculty copies, chapters, etc.)
  • Report to identify fast adds that need cataloger review.
  • Fast add needs to be included at once of the folio default Instance statuses

Need for notes vs. notes app - plain old notes are fine for the short term.
