Updates to "Lost items requiring actual cost" processing page mock-ups

Hi Kimie! Hope all is well with you...

We need to make some changes to the to "Lost items requiring actual cost" processing page mock-ups you created a while back. We have decided that it works best to not allow a billed amount of 0.00, and instead offer a "Do not bill" option.

I have attached the 4 existing mock-ups with the needed changes. I have also attached 2 new mock-ups for the "Do not bill" option (the file names begin with "New_").

Please feel free to change any of the wording as needed. Let me know if you have any questions.

For 3- and 4-, I just needed the option removed, but the field "Additional information for patron" remains (because a notice will be sent).
