Last name should have focus/cursor for newly created user

Description

Open https://folio-snapshot-stable.aws.indexdata.com/users?layer=create&sort=name to create a new user.
Expected: The focus and the cursor are in the required "Last name" input field so that I can start typing.
Actual: No focus, no cursor. When doing keyboard navigation only I need to press Tab three times to move the cursor into the "Last name" input field.

Keyboard navigation is a basic (Level A) accessibility requirement (WCAG 2.1 Criterion 2.1.1).
