Editing fixed due date schedule: New row should be blank

Description

Steps to repro:

  1. Log into folio-snapshot

  2. Go to Settings > Circulation > Fixed due date schedule and create a FDDS with 2 rows

  3. Save (there is a validation bug here UICIRC-338, but if you re-select your inputs and save again, it will let you save)

  4. Now edit the FDDS and add another row

Expected: Row should be blank

Actual: Row is created populated with the same data as the last row

Please write some tests for this when fixing this bug. This was all working at one time and has regressed.

I tried to reproduce it but it seems like everything is working as expected. Perhaps I'm not testing this correctly.
