For a monthly publication, such as Nature Medicine, which has monthly publications which have volume (one per year, continuously increasing) and issue numbers (going from 1-12 in each volume) an example publication pattern might be setup as follows (screen shots first, followed by a textual description of each property and value):this pattern is based on the publication pattern for Nature Medicine / ISSN 1078-8956)

The publication American Fruit Grower / ISSN 0002-8568 is also a monthly publication but with slightly different issue labelling. The publication cycle and enumeration information would be exactly the same as Nature Medicine above, but the template would differ very slightly:

If the publication follows a pattern of being published on a specific weekday (e.g. first Monday of the month) then you would need to choose Day (Mon-Sun), Week (1-4) for this property
