• "Name Format":  "ATTRIBUTE_FORMAT_BASIC"
• "User Attribute Name":  "email"

NOTE: In order for this to work, we need to also set the following on the main "Provider Details" form:

• Principal type:  "Attribute [Name]"
• Principal attribute:  "EmailAddress"
