UX: Proxy labeling refinement

Description

Purpose: To refine the labeling used on the Proxy screens so that it will be more clear to users.

Suggestion:
1. Rename the “Proxy” section to “Proxy/Sponsor”

2. Include the user’s name in the label for each section:
a. <user name> is proxy for:
b. <user name> is sponsor of:

Hi . and I showed the idea of relabeling the proxy section to the RA SIG today (1/18) and they really thought it would make it more clear if we could include the user's name in the label. I've posted an updated screen here:
