Only one Patron Block is displayed in Check-Out when the patron has more than one block

Description

While testing the Q4 2018 release at http://folio-q4-core.aws.indexdata.com on January 9, Holly discovered that when a patron had two blocks, only the first one displayed in the message box. See attached screen prints for details. We will only display the most recent 3 patron blocks. If patron has more than 3 display the message shown in attached screen mock-up if-patron-has-more-than-3-blocks.jpg.

Holly just tested this at https://folio-snapshot.aws.indexdata.com and both renewals via the ellipsis and requests are working perfectly now! Thanks!

Holly tested this at https://folio-snapshot-stable.aws.indexdata.com and it looks good. She couldn't test renewals via the ellipsis because it doesn't seem to be working on snapshot-table. Neither are requests.
