Visible patron ID configuration always includes "User custom fields" selected

Overview:

When configuring Visible patron ID settings, it should e possible to deselect (or not select, at all) User custom fields. However, the current implementation always presents the check box as checked, even if explicitly un-checked and the configuration saved.

Steps to Reproduce:

1. Log in to FOLIO environment with at least one valid central server configuration in INN-Reach

2. Navigate to Settings > INN-Reach > Circulation (mappings) > Visible patron ID

3. De-select the "User custom fields" option

4. Save

5. Reload the settings pane

Expected Results:

"User custom fields" is not selected

Actual Results:

"User custom fields" is selected
