Statistical codes page fails on new entry

Description

Attempting to add a new entry to the Settings > Inventory > Statistical codes page fails with a react error:

Similar lines in ui-users execute without problems but these lines in ui-inventory explode. Weird.

Hi , reg. https://folio-org.atlassian.net/browse/UIIN-385: Scenario no. 4 does not work anymore. When I'm trying to add a new statistical code in Settings / Inventory / Statistical Code by clicking "New" I get the following error: "ERROR:useField must be used inside of a <Form> component". Adding code types works as expected ...

Tested on FOLIO testing.

Yes, this is weird. This is in both FOLIO Snapshot, and FOLIO Snapshot Stable

BUT in the Q3.1. Bugfest environment (https://bugfest.folio.ebsco.com/settings/inventory/StatisticalCodeSettings) all is working as expected
