cannot save new match profile

Overview:
Steps to Reproduce:

1. Log into bugfest.folio.ebsco.com as user "cpu" NOTE: A-M was able to reproduce on folio-testing and folio-snapshot, using Chrome

2. Navigate to Settings>Data Import>Match profiles

3. Select "New" button

4. Provide Name for the Match profile. Description skipped. Could not interact with Details options, so also skipped.

5. Select "Save" button

Expected Results:
New match profile to be created, to be returned to the Settings>Data Import>Match profiles and to have the new profile visible on the screen.
Actual Results:
Get an error message, and am unable to save the match profile
