Cost-per-use does not honor format

Description

See MODEUR-101 . The format parameter is not passed to the backend. This makes the backend choose all journals and books. The other problem (missing titles) is not a UI issue.

Previously I was omitting this parameter and there was comment saying "Not supported on back-end". Whether that was once true but no longer is, or whether it was always a mistake on my part, I'm now sending the parameter, yielding URLs like https://thor-okapi.ci.folio.org/eusage-reports/stored-reports/cost-per-use?accessCountPeriod=1Y&agreementId=3b6623de-de39-4b43-abbc-998bed892025&endDate=2021-10&format=JOURNAL&includeOA=true&startDate=2019-10
