Can not add add accounts to organization from edit form

Overview: Can not add add accounts to organization from edit form. Adding an account produces an interface error

Steps to Reproduce:

1. Log into bugfest-goldenrod

2. Click organizations app

3. Select an organization the is a vendor

4. Click edit

5. Click "add" under the accounts accordion

Expected Results: Clicking add displays a new account field set and user can fill in these fields and save the organization record with new accounts

Actual Results: Error page displays and edit form is dismissed.
