Steps to Reproduce:
1. In Inventory App, open any Instance record (for example, ABA Journal)
2. Go to New Item Page (clicking Add item button) or Edit Item Page (clicking Item: barcode and then Actions button -> Edit).
3. Run "axe". (Screenshot attached).

The problem is
IDs used in ARIA and labels must be unique
How to fix
Fix the following:
Document has multiple elements referenced with ARIA with the same id attribute: input_missing_pieces_date
Acceptance criteria:
Ensure every id attribute value used in ARIA and in labels is unique
