New/Edit Item Page - Accessibility Error: Some elements has insufficient color contrast of 3.3 and several elements with same id

Description

Steps to Reproduce:
1. In Inventory App, open any Instance record (for example, ABA Journal)
2. Go to New Item Page (clicking Add item button) or Edit Item Page (clicking Item: barcode and then Actions button -> Edit).
3. Run "axe". (Screenshot attached).

The problem is:

Some elements has insufficient color contrast of 3.3 and several elements with same id

How to fix:
Fix the following:
Element has insufficient color contrast of 3.3 (foreground color: #888888, background color: #f7f7f7, font size: 10.5pt (14px), font weight: normal). Expected contrast ratio of 4.5:1.

Document has active elements with the same id attribute: clickable-add-note

Acceptance criteria:

Ensures the contrast between foreground and background colors meets WCAG 2 AA contrast ratio thresholds.
Ensures every id attribute value of active elements is unique.
