Adapt mod-ncip to the request schema changes

Description

TLR - data migration - Technical Designs and Decisions - FOLIO Wiki

Only the post request to circulation/requests should be changed. In the module descriptor only the version 12 of circulation interface should be supported. Any missing fields from the list of required request entity properties(find the screenshot attached) should be added. As mod-ncip creates item level requests, it should also contain holdingsRecordId and itemId and set RequestLevel to Item.
