Change instanceId to instance_id for the Item API

Description

The resource bodies described by JSON.Schema need to respect JSON naming conventions.

The instance ID represented in an item used pascal case and so needs changing to use an underscore instead.

I think I might have got confused, I saw some fields that use underscores and thought I'd seen something which confirmed that this was the correct approach. Having looked again, I can't find that and camel case is the most popular it seems.

Marc, I think in Okapi and some other APIs we have been using the CameCase convention. Is underscore conventions preferred in JSON and if so can you link to the spec. I am in favour of CamelCase but consistency is most important.
