Add b-tree index needed for age to lost delayed billing

Description

Needed for CIRC-851.

Add loan b-tree indexes for:

  • agedToLostDelayedBilling.lostItemHasBeenBilled;

  • agedToLostDelayedBilling.dateLostItemShouldBeBilled.

Workaround:
Use a snippetPath .sql file to manually create a custom index name:
