Cate Boerema, you can click the request with return code "500" and see what's the response error message. In this case, I got "Request policy 334e5a9e-94f9-4673-8d1d-ab552863886b could not be found, please check circulation rules" |

Ah, I see. So I checked circ rules and saw the fallback request policy is a policy that doesn't exist. I guess this is a data migration thing because that shouldn't be possible to do via the UI: I will try to manually repair the circ rules and request policies and report back. |

Manually repaired circ rules and was able to create request. Thanks for helping to debug this Hongwei Ji. I will close this issue. |
