TLR: Inconsistent title info loading when request created from Instance record

Overview: Title data is not always loading in the new request form when request is created from Instance record
Steps to Reproduce:

1. Log into some FOLIO snapshot

2. Go to Inventory

3. Select an Inventory record (I used Temeraire) 

4. From the Actions menu, click New request

Expected Results: Title information appears in the request form
Actual Results: It doesn't
Additional Information: I was only able to reproduce this issue once. All other attempts with this title and others the title information loaded in the request form
